Active In-Home Therapy is seeking a Home Health Occupational Therapist to provide client-centered OT services in patients' homes. The role offers a PRN, not-full-time schedule with W2 employment and flexible area coverage.

Responsibilities include assessing patients, creating and implementing personalized treatment plans, and educating families on strategies to improve independence and daily functioning. Collaboration with the care team is essential.

Formación

• Active Occupational Therapist license in the relevant state.
• Experience in home health is a plus but not required.
• Strong communication and interpersonal skills.
• Proficient with technology and electronic documentation.

Responsabilidades

• Conduct thorough assessments of patients' functional abilities to determine rehabilitation needs.
• Develop and implement individualized occupational therapy treatment plans with the healthcare team.
• Provide hands-on occupational therapy interventions to address activities of daily living, fine motor skills, and adaptive techniques.
• Educate patients and families on adaptive strategies, assistive devices, and techniques to enhance independence in daily activities.
• Monitor and document patients' progress, adjusting treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and up-to-date patient records.
• Communicate effectively with patients, families, and caregivers to ensure a positive and supportive therapeutic environment.
