Job summary

Active In-Home Therapy is seeking a skilled Physical Therapist for a PRN role in California. You will join our team as a W-2 employee, not an independent contractor, and provide flexible, as-needed home health services.

You will assess patients, develop individualized plans with the care team, deliver hands-on therapy to improve strength and mobility, educate families, and document progress to ensure quality care.

Qualifications

• California PT license required or in process.
• Home health experience is a plus.
• Excellent communication and organizational skills.
• Proficient with electronic documentation systems.

Responsibilities

• Assess patients and determine rehabilitation needs.
• Develop and implement individualized treatment plans with the care team.
• Provide hands-on PT interventions to improve strength, balance, and mobility.
• Educate patients and families on exercises and safety.
• Monitor progress and adjust plans as needed.
• Document and communicate patient progress with the team.
