Job summary

Hospice of South Texas in Victoria, TX is seeking a compassionate PRN Registered Nurse (RN) to join our clinical team for homecare. The RN will provide skilled hospice care in patients' homes, coordinating with the interdisciplinary team and supporting families through the hospice journey.

Requirements include graduation from an accredited nursing program, Texas RN license, valid driver’s license, CPR, and at least two years of clinical RN experience with hospice exposure.

Qualifications

• Graduation from an accredited Diploma, Associate, or Bachelor School of Nursing.
• Current Texas RN license and driver's license.
• Hospice experience required.

Responsibilities

• Perform comprehensive patient assessments and update care plans.
• Provide direct nursing care and symptom management to patients.
• Coordinate services with physicians, aides, social workers, and chaplains.
• Educate patients and families regarding care needs and disease progression.
• Maintain accurate and timely clinical documentation.
• Promote compliance with hospice regulations and payer requirements.
• Participate in on-call rotation as needed.
