Job summary

Reliant at Home is seeking a PRN Hospice RN Case Manager to cover Tyler and surrounding areas. The role focuses on comprehensive nursing care planning, assessments, and coordination for hospice patients at home.

The ideal candidate has RN licensure, hospice experience preferred, and CPR. You will collaborate with an interdisciplinary team and provide on-call support as needed, ensuring high-quality end-of-life care.

Qualifications

• Graduate of an accredited nursing program (Diploma, ADN, or BSN).
• Current RN license for Texas and CPR certification.
• Hospice experience preferred; home health exposure a plus.
• Two years RN experience in clinical care preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of hospice patients’ needs.
• Determine level of care and eligibility for hospice services.
• Develop and revise the plan of care to ensure quality and continuity.
• Educate patients and families about disease, end-of-life care, and self-care.
• Coordinate services with the IDG and provide staff training as needed.
• Assess caregiver readiness and adapt care based on acuity.
• Provide on-call coverage and clinical direction to peers.
• Document progress notes and communicate changes to the physician.
