Job summary

Cleveland Clinic Fairview Hospital in Independence, OH seeks a PRN RN for Hospice Home Care Services to provide compassionate nursing care in patients' homes. You will implement care plans, coordinate services, and supervise LPNs and family caregivers in a flexible per diem schedule.

Requirements include Ohio RN licensure, BLS certification, and at least 1 year of RN experience, with home health or hospice background preferred.

Qualifications

Responsibilities

• Provide direct nursing care in accordance with policies and procedures.
• Implement and monitor patient care plans.
• Supervise LPNs, Home Care Aides, and family caregivers.
• Perform admission assessments and ongoing reassessments.
• Assess patients for safety risks, abuse, and neglect.
• Coordinate patient care across providers.
• Assess and document patient pain using standard scales.
• Prioritize and document patient problems with patients and families.
• Maintain complete and up-to-date documentation of patient condition.
• Escalate changes in patient condition to the physician as appropriate.

What You'll Do

• Provide direct nursing care in accordance with established policies and procedures.
• Implement and monitor patient care plans.
• Supervise LPNs, Home Care Aides, and family caregivers.
• Perform accurate admission assessments and reassessments.
• Assess patients for safety risks, abuse, and neglect.
• Serve as the primary coordinator for well‑coordinated patient care.
• Assess and document patients' pain using appropriate pain scales.
• Prioritize and document patient problems in collaboration with patients and families.
• Maintain complete and up‑to‑date documentation regarding patient condition.
• Escalate changes in patient condition to the physician as appropriate.

Requirements

Education

• Graduate from an accredited school of Professional Nursing
• Bachelor's of Science in Nursing (BSN) preferred

Experience

• Minimum one year current clinical experience as a registered nurse
• One year of clinical experience as an RN in an acute medical/surgical setting

Licenses & Certifications

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
• Valid Driver's License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage
