Job summary

Crossroads Home Health & Hospice in Vallejo, CA is seeking a licensed Speech-Language Pathologist for per diem home health work, delivering one-on-one therapy in patients' homes. Requires a Master's in Speech-Language Pathology, a CA SLP license in good standing, CCC-SLP or eligible, CPR, a valid driver's license,

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current California SLP license in good standing.
• ASHA CCC-SLP or the education and experience to qualify for it.
• At least one year of clinical SLP experience; home health/geriatric preferred, especially dysphagia.
• CPR certification.

Responsibilities

• Evaluate speech, language, cognition, and swallowing in home health patients.
• Set functional goals and develop each patient’s plan of care.
• Deliver therapy using specialized techniques and equipment.
• Recommend communication devices and aids as indicated.
• Teach patients and caregivers the treatment program for continuity between visits.
• Coordinate with referring physician and interprofessional team.
• Complete clinical documentation accurately and on time.
• Participate in quality improvement activities and peer consultation.
