Sonder Healthcare in Roseville, CA is seeking a per diem Speech-Language Pathologist (SLP) to evaluate, plan, and provide speech therapy services in home health settings. The SLP will collaborate with a care team to support patient recovery and independence, educate families, and ensure high-quality documentation.

The role requires a Master’s or Doctoral degree in Speech-Language Pathology, California SLP license, CPR certification, and strong communication skills.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology.
• Current California SLP license.
• Minimum one year of clinical experience preferred.
• Current CPR certification.
• Strong communication and organizational skills.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Conduct speech-language evaluations and assess patient needs.
• Develop and implement individualized treatment plans.
• Provide speech therapy using appropriate therapeutic techniques and equipment.
• Recommend communication devices and adaptive aids as needed.
• Collaborate with physicians and interdisciplinary team members to ensure coordinated care.
• Participate in case conferences, trainings, and team meetings.
• Maintain accurate and timely clinical documentation.
• Educate patients, families, and caregivers on treatment plans and home programs.
• Identify needs for additional services and make referrals when appropriate.
• Supervise home health aides related to the speech therapy plan of care, when applicable.
• Perform other duties as assigned.
