Job summary

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.

• ASHA CCC-SLP or the education and experience to qualify for it.
• At least one year of clinical SLP experience; home health/geriatric preferred, especially dysphagia.
• CPR certification.

Responsibilities

About the Role
Crossroads Home Health & Hospice is looking for a licensed Speech-Language Pathologist to see home health patients in Vallejo and nearby communities on a per diem basis. You will evaluate and treat adults with speech, language, cognitive-communication, and swallowing needs in their own homes, and you will work with nurses, therapists, and physicians who share your patients. If you want flexible scheduling, one-on-one time with patients, and control over your own day, this role offers that.

What You Will Do

• Evaluate speech, language, cognition, and swallowing, including the home environment and any equipment needs.
• Set functional goals and help develop each patient's plan of care.
• Deliver therapy using specialized techniques and equipment.
• Recommend communication devices and aids when they are indicated.
• Teach patients, families, and caregivers the treatment program so progress continues between visits.
• Coordinate with the referring physician and the interdisciplinary team.
• Take part in case conferences and in-services as needed.
• Complete clinical documentation and progress notes accurately and on time, following agency policy and state and federal regulations.
• Participate in quality assessment and performance improvement activities and in peer consultation.

What You Bring

• A master's degree in Speech-Language Pathology from an accredited program.
• A current California Speech-Language Pathologist license in good standing.
• An ASHA Certificate of Clinical Competence (CCC-SLP), or the education and experience needed to qualify for it.
• At least one year of clinical SLP experience. Home health, SNF, or acute rehab experience with adult and geriatric patients is preferred, especially dysphagia management.
• Current CPR certification.
• A valid driver's license, a reliable vehicle, and auto insurance that meets state and agency requirements.
• Strong verbal and written communication and good organization skills.
