Job summary

Pivot Point Home Care is seeking a licensed Physical Therapist Assistant to deliver home health visits in and around Brockton, MA. This per diem role offers one-on-one patient care with scheduling control and supervision from a PT.

Expect roughly 5-15 visits per week, depending on census and geography. Compensation is $45 per completed visit, with potential efficiency gains when visits cluster geographically.

Qualifications

• Current Massachusetts Physical Therapist Assistant license.
• At least 1 year of PTA experience.
• Home health experience preferred.
• Reliable transportation for patient visits.
• Ability to work independently in the field.
• Strong documentation and communication skills.
• Completion of required background check and onboarding.

Responsibilities

• Provide skilled therapy visits in patients’ homes under the direction of a licensed Physical Therapist.
• Follow the established Plan of Care and help patients work toward functional goals.
• Support patients with mobility, strengthening, balance, transfers, safety, and home exercise programs.
• Educate patients and caregivers on exercises, fall prevention, and safe movement in the home.
• Monitor patient progress and communicate changes or concerns to the supervising PT and care team.
• Complete visit documentation accurately and on time.
• Participate in team communication and required in-service education as needed.
