Job summary

BrightStar Care of Acton–Andover–Lowell is seeking Per Diem Physical Therapists (PT/RPT) to complete assessment-only visits for skilled home health patients. This role offers flexible scheduling, meaningful one-on-one patient care, and limited ongoing treatment responsibilities.

You will conduct initial and follow-up assessments, evaluate patient needs, and establish plans of care while educating patients and caregivers on safety and home exercise strategies.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active MA PT license.
• Home health or skilled nursing experience preferred.
• Ability to travel within the local service area.
• Strong documentation and communication skills.
• Independent work style.

Responsibilities

• Conduct initial and follow-up assessments per physician orders.
• Evaluate patient functional abilities and determine care needs.
• Establish plan of care and provide clinical recommendations.
• Educate patients and caregivers on safety, mobility, and home exercise strategies.
• Complete timely documentation in compliance with state and regulatory standards.
• Collaborate with nursing and care teams to support patient outcomes.
