Job summary

Pivot Point Home Care is seeking a licensed Physical Therapist Assistant to deliver home health visits in and around Brockton, MA. This per diem role offers one-on-one patient care with scheduling control and supervision from a PT.

Expect roughly 5-15 visits per week, depending on census and geography. Compensation is $45 per completed visit, with potential efficiency gains when visits cluster geographically.

Qualifications

• Current Massachusetts Physical Therapist Assistant license.
• At least 1 year of PTA experience.
• Home health experience preferred.
• Reliable transportation for patient visits.
• Ability to work independently in the field.
• Strong documentation and communication skills.
• Completion of required background check and onboarding.

Responsibilities

• Provide skilled therapy visits in patients’ homes under the direction of a licensed Physical Therapist.
• Follow the established Plan of Care and help patients work toward functional goals.
• Support patients with mobility, strengthening, balance, transfers, safety, and home exercise programs.
• Educate patients and caregivers on exercises, fall prevention, and safe movement in the home.
• Monitor patient progress and communicate changes or concerns to the supervising PT and care team.
• Complete visit documentation accurately and on time.
• Participate in team communication and required in-service education as needed.

Job description

Pivot Point Home Care is hiring a per diem Physical Therapist Assistant (PTA) to provide home health visits in and around Brockton, MA.

This is a flexible, per-visit role for a PTA who wants one-on-one patient care, control over their schedule, and the ability to work independently with support from a supervising Physical Therapist.

This is a per diem role with flexible scheduling. Expected volume is approximately 5-15 visits per week, depending on patient census, clinician availability, and service area needs.

Compensation

PTAs are paid $45 per completed visit. Actual hourly earnings may vary based on routing, documentation time, cancellations, and scheduling. When visits are close together geographically, clinicians may be able to complete more than one visit in an hour.
