Job summary

OneWell Health Care in Northampton, PA is seeking a dedicated Per Diem Physical Therapist to provide personalized therapy to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings.

You will assess mobility, develop goal-based plans, and collaborate with caregivers to improve independence and safety. The role offers flexible scheduling and field-based work with a supportive team.

Qualifications

• Doctorate or Master's in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification (preferred or required by policy).
• English proficiency required; bilingual (Spanish) a plus.
• Valid driver's license and willingness to travel to home/community sites.

Responsibilities

• Evaluate mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based therapy in home and community settings.
• Help individuals improve mobility, coordination, and independence.
• Recommend mobility aids and adaptive equipment as needed.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
