Job summary

OneWell Health Care seeks a dedicated Physical Therapist (PT) to provide personalized therapy for individuals with IDD in home and community settings across Pennsylvania. You will evaluate needs, create goal-based plans, and collaborate with caregivers to enhance mobility, strength, safety, and independence.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice (Pennsylvania).
• Current CPR certification (preferred or required per policy).

Responsibilities

• Evaluate mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based physical therapy in home and community-based settings.
• Help individuals improve mobility, coordination, physical function, and independence.
• Recommend mobility aids and adaptive equipment as needed.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
