Job summary

OneWell Health Care is seeking a compassionate Occupational Therapist (OTR/L) to provide personalized therapy to individuals with IDD in home and community-based settings in Pennsylvania.

You will evaluate each individual's functional needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to support greater independence, meaningful participation, and quality of life.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current and valid Occupational Therapist Registration/Licence (OTR/L) in the state of practice.
• Current NBCOT certification and any additional state-specific certifications required for practice.
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency with OT assessment tools, functional evaluation methods, sensory integration, and interventions.
• Willingness to travel to home and community-based service locations; flexible schedules.
• English proficiency; Spanish a plus.

Responsibilities

• Conduct functional, standardized, and observational assessments.
• Develop and update individualized treatment plans.
• Provide evidence-based occupational therapy in home and community-based settings.
• Help individuals strengthen daily living, functional, and adaptive skills.
• Recommend adaptive equipment, assistive devices, and environmental modifications.
• Educate individuals, families, and caregivers on therapeutic strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
