Job summary

OneWell Health Care is seeking a compassionate Occupational Therapist (OTR/L) to provide personalized therapy to individuals with IDD in home and community settings in Pennsylvania. You will evaluate functional needs, develop goal-based treatment plans, and collaborate with caregivers to support greater independence, meaningful participation, and quality of life.

Qualifications

• Bachelor's or Master’s degree in Occupational Therapy from an accredited program.
• Current OT Registration/Licence (OTR/L) in the state of practice.
• Current NBCOT certification and any state-specific certifications required for practice.
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency with OT assessment tools, functional evaluation methods, and EHR systems.
• Valid driver’s license and willingness to travel to home/community sites.
• Experience with IDD in home or community settings preferred.
• English proficiency; bilingual (Spanish) a plus.

Responsibilities

• Conduct functional, standardized, and observational assessments.
• Develop and update individualized treatment plans.
• Provide evidence-based OT in home and community-based settings.
• Help individuals strengthen daily living and adaptive skills.
• Recommend adaptive equipment and environmental modifications.
• Educate individuals, families, and caregivers on therapeutic strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
