Job summary

Premier Home Health Providers Inc. is seeking an Occupational Therapist to support patients at home on a per diem basis, delivering assessments, plans, and training.

You will work with physicians and the care team to maximize independence and safe daily living in the patient's home. Requirements include a CA OT license, NBCOT registration, 2 years of clinical OT experience, CPR (AHA), and the ability to travel to patient homes.

Qualifications

• Graduate of an OT curriculum accredited by the AMA and AOTA.
• Registered with NBCOT and licensed to practice in California.
• CPR certification (AHA) and valid driver's license with insured vehicle.
• Minimum of 2 years clinical OT experience and strong communication.
• Willingness to travel to patient homes and coordinate care with the team.

Responsibilities

• Evaluate patients’ functional abilities and home environments for safety needs.
• Develop personalized OT treatment plans and communicate with physicians.
• Teach and retrain patients in activities of daily living.
• Recommend adaptive equipment and prosthetic/orthotic devices as needed.
• Maintain accurate clinical records and report changes promptly.
• Coordinate with the care team for seamless patient-centered care.
• Educate families and caregivers on treatment approach.
• Supervise OT assistants and home health aides when OT is sole skilled service.
• Support discharge planning as patients transition home.
• Perform other duties to support patient services.
