Job summary

Premier Home Health Providers Inc. is seeking an Occupational Therapist to deliver in-home therapy services on a per diem basis, traveling to patient homes to assess, plan, and implement treatment focused on regaining independence and safe daily living activities.

You will develop personalized plans, educate families, supervise assistants, and contribute to discharge planning while maintaining comprehensive records and collaborating with a multidisciplinary team.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AMA/ AOTA
• Registered with the National Registration Examination for OT
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
