Job summary

There For You Home Care Services is seeking per diem Occupational Therapists to support patients in New Haven, Branford, and Valley areas. Flexible day-rate visits and meaningful one-on-one care are core to this role, with emphasis on independence, home safety, and compliant documentation.

Clinicians will assess, plan, and implement individualized care, train patients on adaptive techniques, and collaborate with an interdisciplinary team to ensure continuity of care and timely discharge planning.

Qualifications

• Proven working experience as occupational therapist in home health.
• Excellent knowledge of OT principles and practices.
• Hands-on experience with documentation and compliance.

Responsibilities

• Conduct home OT evaluations and develop POC with measurable goals.
• Provide skilled OT interventions to improve independence in daily activities.
• Coordinate care with nursing, PT, SLP and physicians.
