Job summary

Day Kimball Health in Thompson, Connecticut seeks a Per Visit Occupational Therapist to join our Homecare team. The OT provides in-home evaluations and develops individualized treatment plans to maximize independence and safety in daily activities.

The role collaborates with physicians, nursing, rehab professionals, social workers, and aides to meet patient-centered goals while maintaining compliant documentation and scheduling flexibility.

Qualifications

• Graduate of an accredited OT program with a 4-year/advanced degree.
• Currently licensed as an Occupational Therapist in Connecticut.
• Current BLS/CPR certification.
• Valid CT driver’s license and reliable transportation.

Responsibilities

• Perform home-based OT evaluations.
• Develop and implement individualized treatment plans.
• Provide skilled OT interventions to improve independence, safety, and ADLs/IADLs.
• Educate patients and families on treatment plans and home programs.
• Collaborate with interdisciplinary team to achieve patient goals.
• Maintain accurate clinical documentation.
• Evaluate progress and adjust goals as needed.
