Job summary

Northeast Healthcare Recruitment seeks Nurse Practitioners to perform in-home health assessments across Cape Cod, Massachusetts. This per diem role offers independence, competitive pay per assessment, and mileage reimbursement for travel.

Qualifications

• Active Nurse Practitioner license in Massachusetts.
• Board certification from AANP or ANCC in Family, Adult-Gerontology, or related specialty.
• Minimum of 1 year of clinical experience, preferably with geriatric patients.

Responsibilities

• Perform in-home health assessments, review medical histories, and conduct physical exams.
• Educate patients on medication management and disease prevention.
• Document diagnoses and communicate findings for care management.
• Collaborate with primary care providers to address gaps in patient care.
• Maintain flexible, self-scheduled work hours with travel to Cape Cod.
