Qualifications

• Active Nurse Practitioner license in Kansas (required).
• Board certification in Family, Adult-Gerontology, or related specialty.
• Minimum 1 year clinical experience, geriatric preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Perform in-home health assessments reviewing medical history and conducting exams.
• Educate patients on medications, disease prevention, and health maintenance.
• Document diagnoses and communicate findings for care management.
• Collaborate with primary care providers to address gaps in care.
• Work with flexible scheduling; no on-call weekends or holidays required.
