Qualifications

• Active NP license in Massachusetts.
• Board certification from AANP or ANCC in Family, Adult-Gerontology, or related specialties.
• Minimum of 1 year clinical experience, preferably geriatrics.
• Availability to complete at least 30 in-home assessments per month.
• Reliable transportation for local travel.
• Comfort working in varied home environments.

Responsibilities

• Perform in-home health assessments, review medical histories, conduct exams, screen for chronic conditions.
• Educate patients on medication management, disease prevention, and overall health maintenance.
• Document diagnoses and communicate findings for care management.
• Collaborate with primary care providers to close gaps in patient care.
• Offer flexible, self-scheduled work hours (no on-call, weekends, or holidays).
