Qualifications

• Active LPN license (OH or Compact)
• Minimum 1 year experience in Skilled Nursing Facility
• Reliable transportation to assigned facilities
• Consistent attendance and punctuality
• Ability to push, pull, and lift at least 50 pounds
• Ability to stand and walk with frequent bending, reaching, and lifting with proper body mechanics
• Legally authorized to work in the United States
• Willing to complete background and drug screening as required by law

Responsibilities

• Record patients’ medical information and vital signs in the EMR
• Administer medications and treatments per physician orders and facility policy
• Monitor resident conditions and report significant changes to the clinical team
• Document medications, treatments, observations, and resident responses
• Provide direct nursing care including wound care within scope of practice
• Assist with resident assessments and care plan implementation
• Coordinate with CNAs and healthcare team to ensure quality care
• Maintain compliance with infection control and regulatory requirements
• Communicate concerns to onsite supervisor and agency
• Other duties as assigned
