Job summary

Chapters Health System is seeking a Per Diem RN to assess and address patient and family needs, coordinating the Plan of Care with the IDG and providing compassionate palliative care.

The role requires current RN licensure, BLS, and at least one year of nursing experience, with EMR/EHR familiarity. A valid driver’s license and reliable transportation are essential for visiting patients across multiple sites.

Qualifications

• Current license as RN in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year nursing experience; hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to manage emotional stress of caring for terminally ill patients and families.
• Physical ability to navigate stairs and various environments and perform duties with/without accommodation.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG).
• Coordinates the Plan of Care ensuring it reflects evolving patient needs.
• Educates patient, family, caregivers about disease, pallaitive care and safety.
• Reports changes in patient condition to IDG or other health professionals.
• Participates with the IDG to evaluate hospice referrals/admissions for level of care.
• Attends patient deaths and prepares body for transport; oversees disposal of medications.
• Maintains accurate documentation and communicates with physicians and teams.
