Job summary

Chapters Health System in the Washington, DC area seeks a Per Diem RN to provide compassionate, patient- and family-centered hospice care within the Interdisciplinary Group. Qualifications

• Current RN license in the state where the employee will be working.
• Active BLS for healthcare professionals.
• Minimum one (1) year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to use equipment with visual and auditory mechanisms.
• Ability to effectively communicate in English (verbal and written).
• Flexible schedule to meet the needs of Hospice House patients.
• Ability to manage the emotional stress of working with terminally ill patients and their families.
• Ability to physically access patient rooms and other areas; lift/push/pull as needed.
• PACE locations may require Alzheimer's Disease and Related Dementias Training.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG).
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient and family about disease, palliative care, safety and caregiving.
• Reports changes in the patient’s condition to IDG or other health professionals.
• Evaluates hospice referrals/admissions for level of care appropriateness.
• Attends patient deaths and prepares body for transport; oversees disposal of medications.
• Provides bereavement interventions to the family and caregiver with psychosocial staff.
• Presents concise oral and written reports to IDG and respects input from all disciplines.
• Communicates with physicians, staff, patients and families; collaborates with teams.
• Supervises nursing team members; coordinates care across shifts.
• During emergencies, may report to a designated location to ensure continuity of services.
• Performs other duties as assigned.
