Job summary

Chapters Health is seeking Per Diem Registered Nurses to assess patient and family needs, coordinate care with the Interdisciplinary Group, and provide compassionate palliative support across hospice settings. The role requires an active RN license, BLS certification, and experience with EMR/EHR systems.

Ideal candidates will have flexibility in scheduling, strong English communication skills, and reliable transportation to meet visit schedules in the Florida area.

Qualifications

• Active RN license in the state of employment.
• BLS for healthcare professionals from AHA or Red Cross.
• 1 year inpatient nursing or hospice experience; internal affiliates may have 6 months LPN in hospice/home health.
• Experience with EMR/EHR systems.

Responsibilities

• Provide direct patient/family care within the IDG framework.
• Coordinate Plan of Care and ensure timely updates.
• Educate patients and families on disease, palliative care, and safety.
• Perform nursing interventions to prevent complications and maximize comfort.
• Report changes to IDG and physicians; participate in admissions as needed.
