Job summary

Chapters Health System is seeking a Per Diem RN to assess patients and coordinate the Plan of Care within a hospice setting. The role emphasizes compassionate, patient-centered care and collaboration with the Interdisciplinary Group to support patients and families.

Responsibilities include nursing assessments, care planning, education for patients and families, and ensuring adherence to regulatory guidelines while delivering high-quality end-of-life care.

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals from AHA or Red Cross.
• Minimum one year of inpatient nursing or hospice experience for external candidates; internal applicants require 6 months of LPN hospice/home health within an affiliate.
• Experience with EMR/EHR systems.

Responsibilities

• Provide direct care to patients and families as part of the IDG with palliative care.
• Coordinate the Plan of Care reflecting patient needs.
• Educate patients and families on disease, comfort, and safety measures.
• Report changes in patient condition to IDG and physicians.
• Participate in admissions and bereavement activities as appropriate.
