Job summary

CenterWell Home Health is seeking a per diem Speech-Language Pathologist to deliver in-home care, evaluating patients and guiding rehabilitation to improve communication, swallowing, and overall function. You will work under the Clinical Manager, with a territory-based caseload and flexible per-diem hours.

The role requires ASHA CCC-SLP or Audiology certification, current licensure, CPR, and a valid driver’s license with reliable transportation. Per visit compensation ranges from $54 to $76.

Qualifications

• Must have CCC-SLP or Audiology certification (ASHA).
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license required.
• Current CPR certification required.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Evaluate and provide speech-language pathology services in patients' homes or facilities.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Document patient changes and progress and report to supervisor/physician.
• Instruct patients in alternative communication systems when appropriate.
• Counsel patients, families, and healthcare staff; maintain required documentation.
