Qualifications

• Meet the education and experience requirements for ASHA CCC in Speech Language Pathology or Audiology.
• Minimum six months experience as a speech therapist or SLP.
• Current and unrestricted license and CPR certification.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in development and periodic review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to improve function.
• Administer and interpret diagnostic tests and applications of therapy.
• Record and report changes in patient condition to supervisor or physician.
• Provide instruction to patients in alternative communication systems as appropriate.
• Counsel patients, families and healthcare staff; maintain required documentation.
• Participate in care coordination and discharge planning.
