CenterWell Home Health is seeking a Per Diem Speech Language Pathologist to deliver home-based SLP services, focusing on rehabilitation and functional communication goals. You will assess, treat, and document patient progress in collaboration with the Clinical Manager and care team.

Responsibilities include developing and reviewing plans of care, administering diagnostic tests, and educating patients and families. Requires a valid SLP license, CPR, and transportation for in-home visits.

Formación

• CCC-SLP certification or ASHA credential required
• Minimum six months experience as a speech-language pathologist
• Current and unrestricted SLP license
• Current CPR certification
• Valid driver’s license with insurance and reliable transportation

Responsabilidades

• Evaluate, direct and provide speech-language pathology service to patients in home or facility
• Participate in development and review of Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostics and therapeutic applications
• Observe, record and report changes to supervisor/physician
• Provide instruction on alternative communication systems when appropriate
• Counsel patients, families and healthcare staff
• Maintain documentation and submit reports per company policy
• Participate in care coordination and discharge planning
• Maintain confidentiality of patient information

Conocimientos

Six months experience as SLP/SLP

CPR Certification
