Qualifications

• ASHA CCC required for SLP/AuD.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license to practice.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services in the home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapy.
• Observe, record and report changes in patient conditions to supervisor/physician.
• Instruct patients in use of alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain and submit documentation and reports as required by policy.
• Participate in care coordination and discharge planning.
• Maintain confidentiality of patient information.
