Zusammenfassung

PACS Group is seeking a Speech-Language Pathologist (SLP) for All Saints Sub Acute and Skilled Nursing in San Leandro, CA. This part-time or PRN role focuses on improving residents' communication, cognition, and swallowing safety while supporting independence and quality of life.

You will assess, plan, and implement evidence-based treatments, collaborate with a multidisciplinary team, and educate residents and families.

Qualifikationen

• Current and active California Speech-Language Pathologist license.
• Master's degree in Speech-Language Pathology from an accredited program.
• Experience in a skilled nursing, subacute, or post-acute rehabilitation setting preferred.
• Knowledge of speech, language, cognitive-communication, and swallowing disorders.
• Strong clinical assessment, treatment planning, and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with residents, families, and an interdisciplinary care team.
• Compassionate, resident-centered approach to rehabilitation.

Aufgaben

• Evaluate residents to assess speech, language, cognitive-communication, and swallowing needs.
• Develop, implement, and update individualized treatment plans.
• Provide evidence-based speech therapy interventions to improve communication, cognition, swallowing safety, and independence.
• Monitor resident progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, and therapy team on goals and discharge planning.
• Educate residents, families, and caregivers on treatment goals and strategies.
• Provide in-service training to facility staff on speech therapy topics.
• Supervise clinical fellows and students as applicable.
