Job summary

All Saints Sub Acute and Skilled Nursing in San Leandro, CA is hiring a Speech-Language Pathologist (SLP) for a flexible part-time or PRN role on our rehabilitation team. The SLP will evaluate residents, develop individualized plans, and provide evidence-based therapies to improve communication, cognition, and swallowing.

Collaboration with physicians, nurses, and families is essential to discharge planning and quality care.

Qualifications

Current and active California Speech-Language Pathologist license.
Master's degree in Speech-Language Pathology from an accredited program.
Experience in skilled nursing, subacute, or post-acute rehabilitation settings preferred.
Knowledge of speech, language, cognitive-communication, and swallowing disorders.
Strong clinical assessment, treatment planning, and documentation skills.

Responsibilities

Evaluate residents to assess speech, language, cognitive-communication, and swallowing needs.
Develop and update individualized treatment plans based on goals.
Provide evidence-based speech therapy interventions to improve communication, cognition, swallowing safety, and independence.
Monitor progress and adjust treatment plans as needed.
Collaborate with physicians, nursing staff, and families regarding goals, progress, and discharge planning.
Educate residents, families, and caregivers on treatment goals and strategies.
Maintain accurate clinical documentation and ensure regulatory compliance.
