OneWell Health Care seeks a compassionate Speech-Language Pathologist to provide personalized therapy to individuals with IDD in home and community settings across Pennsylvania.

You will assess communication and swallowing needs, develop individualized treatment plans, and collaborate with families and a multidisciplinary team to improve communication, independence, and quality of life for clients. This per-diem, field-based role offers flexible scheduling.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Current and valid SLP license in PA.
• CCC-SLP certification from ASHA required.
• Strong clinical, communication, documentation, and organizational skills.
• Valid driver's license and willingness to travel to home/community sites.
• Experience with IDD and home/community-based services preferred.
• English proficiency required; Spanish a plus.

Responsabilités

• Conduct speech, language, cognitive-communication, AAC, and swallowing evaluations.
• Develop and update individualized treatment plans.
• Provide evidence-based speech-language therapy.
• Collaborate with families, caregivers, and interdisciplinary team members.
• Educate caregivers on communication strategies and safe swallowing practices.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
