Job summary

OneWell Health Care in Pennsylvania seeks a compassionate Speech-Language Pathologist to provide personalized therapy to individuals with IDD in home and community-based settings. You will assess communication and swallowing needs, develop treatment plans, and collaborate with families and interdisciplinary teams to improve communication, independence, and quality of life.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and valid Speech-Language Pathologist license in the state of practice (PA).
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
• Strong clinical, communication, documentation, and organizational skills.
• Valid driver's license and ability to travel to home and community-based service locations.
• Experience with IDD, home or community-based services, and AAC or dysphagia management preferred.
• English proficiency required; bilingual (e.g., Spanish) is a plus.

Responsibilities

• Conduct speech, language, cognitive-communication, AAC, and swallowing evaluations.
• Develop and update individualized treatment plans.
• Provide evidence-based speech, communication, AAC, feeding, and swallowing therapy.
• Collaborate with families, caregivers, and interdisciplinary team members.
• Educate caregivers on communication strategies, AAC use, and safe swallowing practices.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
