Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain independence in the comfort of their own homes. Baywood Home Care offers a part-time, in-person role in Fairmont, MN with pay of $37.44–$45.00 per hour, mileage reimbursement, and access to a company vehicle for business use.

Qualifications

• Current Minnesota RN license required.
• Experience in home health, case management, or transitional care.
• Strong assessment and communication skills with ability to coordinate care.
• Must be 21 years of age or older.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Coordinate services with physicians, therapists, caregivers, and community resources.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on medications, treatment plans, and wellness strategies.
• Maintain accurate clinical documentation and regulatory compliance.
