Job summary

Baywood Home Care is seeking an RN Case Manager to coordinate patient care in the home setting in Fairmont, MN. This part-time, in-person role offers flexible scheduling up to 20 hours per week with compensation based on experience.

Qualifications

• Current Minnesota RN license required.
• Home health, case management, or transitional care experience preferred.
• Strong assessment, communication, and care coordination skills.
• At least 21 years old.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Coordinate services with physicians, therapists, caregivers, and community resources.
• Monitor patient progress and adjust care plans as needed.
