Baywood Home Care in Minnesota is seeking a compassionate Registered Nurse Case Manager to coordinate care for patients and families, supporting independence in the home. This part-time role offers a flexible schedule and meaningful one-on-one patient relationships.

You will conduct assessments, develop care plans, coordinate services with physicians and therapists, monitor progress, and educate patients on medications and wellness strategies, while maintaining compliant documentation.

Qualifications

• Current Minnesota RN license required.
• Home health, case management, or transitional care experience required.
• Strong assessment, communication, and care coordination skills.
• At least 21 years old.

Responsibilities
