Telos Health Systems is seeking a Psychotherapist (LMHC, LCSW, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities.

This part-time, fee-for-service role offers clinical autonomy, flexible scheduling, and strong administrative and leadership support across two locations in Palm Bay and Stuart, FL.

Qualificações

• Active Florida license as LMHC, LMFT, or Licensed Psychologist (PsyD/Ph.D.).
• Master’s or Doctoral degree in a relevant mental health field.
• Ability to work independently while collaborating within a clinical team.
• Strong communication and clinical documentation skills.
• Level II background check required per AHCA.

Responsabilidades

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with the care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and other healthcare professionals.
• Adhere to federal, state, and organizational standards and policies.
