Job summary

Superior Hospice, part of the Home Health Care, Inc. family, seeks an experienced Hospice Visit Nurse (RN) for a part-time role with potential to transition to full-time as patient volume grows. You will provide routine hospice visits in patients' homes and care settings, perform assessments, and educate patients and families.

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred, or strong oncology/palliative/home health experience.
• Excellent clinical assessment and communication skills.
• Ability to work independently in a field-based role.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Provide routine and PRN hospice visits in patients' homes, assisted living, memory care, and skilled nursing settings.
• Perform comprehensive nursing assessments, symptom management, medication education, and comfort-focused interventions.
• Support patients and families through compassionate communication and ongoing education.
• Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate and timely EMR documentation while maintaining high clinical standards.
