Superior Hospice, part of Home Health Care, Inc., is seeking a Hospice Visit Nurse (RN) for a part-time role in Saint Paul. You will provide routine hospice visits in patients' homes, complete nursing assessments, symptom management, and comfort-focused care, with a focus on compassionate communication and supporting families.

Qualifications

• Active Minnesota RN license required.
• Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, med-surg, or end-of-life care.
• Excellent assessment, communication, and time-management skills.

Responsibilities

• Provide routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing settings.
• Complete nursing assessments, symptom management, medication education, and comfort-focused interventions.
• Support patients and families through education, advocacy, and compassionate communication.
• Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.
