Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients with daily living and personal care in their homes, following an established care plan.

You will perform baths, grooming, mobility support, medications reminders, and safety practices while documenting observations and communicating with the supervising nurse. This part-time role offers flexible hours and competitive pay, with opportunities to impact patients’ quality of life.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.
• Must meet all federal, state, and local requirements

Responsibilities

• Assist patients with plan of care including baths, hygiene, dressing and toileting, and feeding.
• Take and record temperatures, pulse, respiration and blood pressure when ordered.
• Support safe transfer and ambulation techniques.
• Perform exercises according to the plan of care and ROM when directed.
• Perform simple procedures as ordered under supervision.
• Assist with patient laundry and safety practices.
• Observe and document patient condition and report changes to the nurse supervisor.
• Communicate clearly with client, family/caregiver and office.
