The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living assistance and personal care for patients in their homes according to an established care plan.

Responsibilities include assisting with baths, hygiene, mobility, vital signs when ordered, and following the plan of care. Must maintain CPR certification and communicate effectively with clients and the office to ensure quality care.

Qualifications

• CPR certification is required.
• Ability to read, follow written instructions and document care.
• Effective communication with clients, families, and supervisors.

Responsabilités

• Assist patients with daily living activities per the care plan (baths, oral hygiene, dressing, toileting, feeding).
• Take and record vitals when ordered (temperature, pulse, respiration, blood pressure) as competency allows.
• Support safe transfer, ambulation, and range of motion as prescribed.
• Provide exercises per the care plan and assist with extensional therapies as ordered.
• Perform patient laundry and ensure home safety; observe and report changes to the nurse supervisor.
