Job summary

The Lakes Home Care, Inc. seeks a compassionate Home Health Aide to provide daily living and personal care for patients in their homes under an established care plan.

The role emphasizes empathy, accuracy in care tasks, and clear communication with families and supervisors. You will assist with baths, grooming, mobility, and basic therapeutic activities while documenting care and reporting changes to the assigned nurse.

Qualifications

• Read and follow written instructions and document care.
• Must have CPR certification.
• Complete Maxim screening examinations and competency program.

Responsibilities

• Assist with activities of daily living per care plan (baths, oral hygiene, dressing, toileting, feeding).
• Take and record vitals when ordered (temperature, pulse, respiration, blood pressure).
• Assist with safe transfers and ambulation.
• Follow safety practices and use equipment properly in home settings.
• Observe and report patient condition to supervising nurse.
