Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established care plan.

You will assist with baths, grooming, feeding, and safety while monitoring patient status and documenting changes for the supervising nurse. Responsibilities include assisting with transfers, range of motion, and following the care plan, with a focus on compassionate, patient-centered care.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements

Responsibilities

• Assist patients with baths, oral hygiene, dressing, toileting, feeding, etc. according to the plan of care
• Take and record vital signs and measurements as ordered (temperature, pulse, respiration, blood pressure)
• Perform safe transfers and ambulation per care plan
• Support exercise as directed by the plan of care
• Carry out ROM and simple procedures as assigned
• Handle patient laundry and maintain household safety
• Observe and document patient condition and report changes to nurse supervisor
