Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established plan of care.

Responsibilities include bathing, dressing, feeding, and monitoring vital signs, with an emphasis on safe transfers, documentation, and clear communication with families and the supervising nurse. Flexible, part-time hours offered.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Must maintain current CPR certification as required by state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with daily living per care plan: baths, hygiene, dressing, feeding.
• Take and record vital signs and report changes to nurse supervisor.
• Provide safe transfer and ambulation support; assist with prescribed exercises.
• Perform range of motion and simple procedures as ordered with competency.
• Handle patient laundry and ensure safety with equipment.
• Observe and teach home safety practices; respond to emergencies.
• Follow plan of care and complete all assigned tasks.
• Communicate clearly with client, family/caregiver and office.
