Job summary
The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established care plan. You will perform baths, hygiene, feeding, and safety monitoring while documenting changes for the supervising nurse.

Qualifications
- Meets all training requirements per state and federal laws.
- Reads and follows written instructions and documents care.
- Current CPR certification is required.
- Successful completion of Maxim screening examinations.
- Participation in Maxim competency program.
Responsibilities
- Assist patients with bathing, dressing, toileting, and personal care per care plan.
- Record and report oral, axillary temperature and other vital signs as ordered.
- Provide safe transfer and ambulation assistance.
- Support exercise and mobility as directed by the plan of care.
- Document care and communicate with the supervising nurse.
- Maintain safety practices and home safety measures.
