Home Care Network, Inc. is seeking a caregiver/home health aide to support clients in Freeport, OH. The role includes direct patient care under RN/therapy direction, adherence to plans of care, and modest travel within the service area.

Ideal candidates have a CNA or STNA credential (or 2+ years of home health experience), reliable transportation, and a compassionate, detail-oriented approach. Flexible PT schedule with weekly pay and growth opportunities are offered.

Qualifications

• Active CNA or STNA credential plus 1 year of paid home health experience.
• Or 2+ years of paid home health experience for HHA or PCA without certification.
• Additional certifications (medication, Hoyer lift, disease-specific care) are desirable but not required.

Responsabilités

• Deliver direct patient care and support following the plan of care.
• Measure and record vital signs as instructed.
• Assist with activities of daily living (bathing, hygiene, dressing).
• Perform household tasks essential to home care (meal prep, laundry, light housekeeping).
• Assist with medications under RN supervision; may handle containers/pill caddies.
• Identify abnormal findings and report changes in patient condition.
• Support therapy exercises under supervision of therapists.
