Job summary

SYNERGY HomeCare of Central IL is seeking caring individuals to join our team as Home Caregivers. You will provide one-on-one in-home care to clients in private residences or care facilities, following each client’s care plan and staying with your assigned client during shifts.

Responsibilities include companionship, personal care, mobility assistance, meal prep, light housekeeping, and medication reminders.

Qualifications

• Must be at least 17 years of age.
• Must possess a valid driver’s license.
• Must have a reliable vehicle.
• Must maintain current automobile insurance.
• Must complete all required Illinois Department of Public Health (IDPH) background check requirements.
• Must be able to work independently and follow established care plans and instructions.
• Must demonstrate strong organizational and time-management skills.
• Must be able to accurately document services provided, observations, and other required information.
• Must be able to lift at least 25 pounds and safely perform the physical responsibilities associated with assigned caregiving duties.
• Must be comfortable working with individuals with a variety of personalities, needs, abilities, and backgrounds.
• Must demonstrate strong verbal and written communication skills.
• Must be dependable, professional, and able to maintain appropriate boundaries with clients.
• Must maintain client confidentiality and privacy.
• Must be able to follow company policies, safety procedures, and infection-control practices.
• Must be able to recognize and appropriately report changes in a client’s condition, safety concerns, incidents, abuse, neglect, or other concerns.
• Must successfully complete all required orientation, training, and competency requirements

Responsibilities

• Provide one-on-one care and support to clients in their homes.
• Work independently with assigned client and follow care plan.
• Communicate concerns to supervisor and provide dependable, professional care.
• Complete day-to-day responsibilities without direct, on-site supervision.
• Provide companionship and social interaction.
• Assist with personal care, including bathing, grooming, dressing, toileting, and activities of daily living.
• Help with mobility, transfers, and walking assistance.
• Prepare meals and assist with eating as needed.
• Perform light housekeeping to maintain a clean, safe home environment.
• Provide medication reminders.
• Assist with errands.
• Observe and report changes in condition.
• Arrive on time, stay for scheduled shift, and complete assigned responsibilities.
