Job summary

First Choice Home Health & Hospice is seeking an experienced On-Call Registered Nurse (RN) to provide after-hours support for home health and hospice patients in Salt Lake County. This Part-Time / On-Call role features a rotating schedule designed for experienced nurses seeking flexible income opportunities.

You will respond to after-hours calls, assess patients remotely, and perform targeted home visits as needed.

Qualifications

Current Registered Nurse (RN) license in good standing.
2+ years of direct patient-care nursing experience.
Strong nursing assessment and clinical decision-making skills.
Excellent communication skills.
Comfortable working independently in patients' homes.
Valid driver's license and reliable transportation.
Ability to travel throughout Salt Lake County during assigned coverage.
Dependable and responsive during your on-call rotation.

Responsibilities

Respond to after-hours patient and caregiver calls.
Assess patient needs by phone and determine when an in-person visit is appropriate.
Complete home health and hospice nursing visits as needed.
Assess changes in patient condition.
Provide symptom management and nursing interventions according to the plan of care.
Administer medications and treatments as ordered.
Educate and support patients, families, and caregivers.
Communicate changes in condition to the clinical team.
Coordinate with physicians and other providers when needed.
Complete accurate and timely clinical documentation.
Support hospice patients and families during urgent or end-of-life situations.
